The nurse will print and provide you with a hard copy at the end of the appointment.<\/p>\n<p><a href=\"\/wp-content\/uploads\/nj-universal-child-health-record.pdf\" target=\"_blank\" rel=\"noopener\">NJ Universal Child Health Record <i class=\"fas fa-external-link\"><\/i><\/a><\/p>\n<p><a href=\"\/wp-content\/uploads\/fare-food-allergy.pdf\" target=\"_blank\" rel=\"noopener\">FARE Food Allergy & Anaphylaxis Emergency Care Plan <i class=\"fas fa-external-link\"><\/i><\/a><\/p>\n<p><a href=\"\/wp-content\/uploads\/asthma-treatment-plan.pdf\" target=\"_blank\" rel=\"noopener\">Asthma Treatment Plan <i class=\"fas fa-external-link\"><\/i><\/a><\/p>\n<p><a href=\"\/wp-content\/uploads\/physical-examination-form.pdf\" target=\"_blank\" rel=\"noopener\">Preparticipation Physical Evaluation<br \/>\nPhysical Examination Form <i class=\"fas fa-external-link\"><\/i><\/a><\/p>\n<p>The forms above will be valid for one year after your child\u2019s annual checkup. Only one copy will be provided to you for the duration of the year.<br \/>\nPlease keep the original with you, and submit copies to the school as necessary.<br \/>\nThe fee for reissuance in case of loss of the original, or any request to change\/add to the content of the original will be $25.<\/p>\n<p>If you require any form not listed above, the fee will be $25. Additional forms may take up to 1 week from the time of request.
