Effective immediately, the following forms will be available for you to take home on the day of your child’s annual checkup appointment.
Please let the nurse know which form(s) you will need for your child. The nurse will print and provide you with a hard copy at the end of the appointment.

FARE Food Allergy & Anaphylaxis Emergency Care Plan
Preparticipation Physical Evaluation
Physical Examination Form
The forms above will be valid for one year after your child’s annual checkup. Only one copy will be provided to you for the duration of the year.
Please keep the original with you, and submit copies to the school as necessary.
The fee for reissuance in case of loss of the original, or any request to change/add to the content of the original will be $25.
If you require any form not listed above, the fee will be $25. Additional forms may take up to 1 week from the time of request. We will notify you when the form is ready (pickup or email options are available).
